Emergence of Venous Stenosis as the Dominant Cause of Pulsatile Tinnitus

Background The role of arteriovenous shunts such as dural fistulas, arterial steno?occlusive states, anatomic variants, and hypervascular tumors in the genesis pulsatile tinnitus (PT) has long been recognized. On venous side, diverticula, high?riding jugular bulb, sinus wall dehiscence have also implicated. However, overall most common cause—venous stenosis (VSS)—continues to be underrecognized. Its clinical importance, separate from association with intracranial hypertension, requires emphasis. Methods A retrospective review last consecutive 208 cases PT seen at our institution was performed cause determined, when possible, based on radiographic data. Results VSS (34% cohort). Over 90% are women. Typical presentation a unilateral whoosh?like sound sync heartbeat that could completely or nearly abolished by ipsilateral compression. This scenario virtually guaranteed presence VSS, very high sensitivity, specificity, positive, negative predictive values. About two thirds patients harbored other variations bulb diverticulum, should not misinterpreted primary PT. Most did signs symptoms even though cerebrospinal fluid pressures frequently elevated.
